Forms

Our forms are stored in Adobe's portable document format (PDF). To view and print them, you will need Adobe's Acrobat Reader. Several of these forms can be filled in while displayed in the browser. This requires a minimum of Acrobat Reader 3.0.

Just click on the name of a form to download it. Some browsers will automatically open the file for you, but others will ask you to save it on your computer, in which case you'll have to open it in the Acrobat Reader yourself. If you need to download Adobe Acrobat Reader, click here.

Advanced Standing Certificate 8/03 Used only for Agricultural Institute students or for transfer students from international institutions. ASC's are sent to Registration & Records (Box 7313) and must be accompanied by the transcript. All other transcripts must be sent to Admissions for evaluation.

FERPA - Parental Access

*identity of student must be verified in person by a university official*

9/07 Allows students to grant guardian access to educational records; the student may also use MyPack Portal through the "Personal Info" menu option to grant parental access.
