Should an error occur

• Document the error with the source, the reporter(s) and editor(s) involved.
• Determine whether a clarification or correction is warranted.
• Publish a clarification or correction as soon as possible, usually the next day.
• Follow up with a phone call and written communication from the editor to the source(s) in question.
